Snoqualmie Valley Hospital is required by law to maintain the privacy of your health information, to provide you with a notice of our legal duties and privacy practices, and to follow the information practices that are described in this Privacy Notice: Notice of Privacy Practices
You have the right to receive a copy of your health information that we maintain, with some limited exceptions. You have the right to receive a copy of your health information in a format you prefer (e.g., paper, email, CD). You have the right to request that your health information to be sent to any person or entity.
Obtain Immunization Records Online with MyIR
Access your Immunization records through MyIR without an extra trip to your health care provider’s office. MyIR allows you to manage your family’s immunization records securely online. Once you register, you can access the records any time you need them. You may also print your child’s Certificate of Immunization Status (PDF) for school and childcare entry. Visit the Washington Department of Health’s website for additional information.
Medical Records Requests
Our Medical Records are managed securely through Verisma. Verisma is not located on the SVH Campus, but they can be reached by emailing snoqualmiehealth@verisma.com. Verisma can provide you with copies of your medical records. You can easily fill out your patient request form online. This form can only be completed by a patient. Attorneys and third-parties cannot request patient records using this online form.
There are a variety of ways you can obtain your record from Snoqualmie Valley Health. The preferred methods are via fax and email.

Our office is open Monday through Friday from 9 a.m. to 4:30 p.m. We are closed for all major holidays.
Processing Time
Costs
For medical use, there is no fee if records are to be sent directly to a doctor or other healthcare provider for the purpose of continuing care.
For copies for patients or their representatives, there may be a reasonable, cost-based fee.
Delivered Electronically: $6.50
Printed: $0.07 per page copying cost + $0.05 per page printing cost.
Amendments
You have the right to request that your health information be amended. If you see documentation in your record that you believe needs correction, please complete the medical record amendment form.
Death/Deceased Notification
How to Notify Snoqualmie Valley Health of Your Loved One’s Passing
We are deeply sorry for your loss. We understand that notifying others about the passing of your loved one can be difficult. Informing Snoqualmie Valley Health when a death occurs outside of our care allows us to update the patient’s medical records and notify the appropriate departments. This helps prevent any unnecessary communication that could inadvertently cause you further distress.
We kindly ask for a copy of the death certificate or another legal document serving as proof of their passing. Please contact info@snoqualmiehospital.org to initiate this process.
Billing
If a deceased patient has an outstanding balance, please request the Estate Administrator, whether a Personal Representative or their Attorney, to send a Creditor’s Claim to:
