Compliance Reporting
A toll-free, continuously managed, compliance hotline is available to receive anonymous concerns regarding compliance matters (1-855-387-4416). A non-anonymous email is also available. Click here to email compliance. Any concerns received by the Hospital’s Compliance Officer via phone or email will be logged into the Hospital’s event management system as are events that are reported from staff members who have identified or are self-reporting deviations from policy or other risk events.
Public Records Requests
King County Public Hospital District No. 4 will make public records of the district available in compliance with the Washington Public Records Act (RCW 42.56) and any other applicable provisions of federal or state law. The district will acknowledge the request within five business days and either make requested records available for inspection, provide copies, or give a reasonable estimate of when the information may be available.
Form 1095-C Availability Notice
In accordance with IRS guidance, Snoqualmie Valley Health will not automatically mail Form 1095-C to employees and covered individuals. Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available upon request. Individuals may request a copy of their Form 1095-C by contacting:

Once requested, the Form 1095-C will be furnished within 30 days of the date the request is received. For additional information, please refer to guidance from the Internal Revenue Service (Notice 2025-15).
